Athletic Training Forms

In order to be medically cleared to participate in Viterbo Athletics, an online medical history questionnaire needs to be completed and submitted to the Sports Medicine Staff. The online medical history questionnaire is required of every athlete, every year. This form allows us to better understand you and your health and allows us to monitor any changes in your health status during your time at Viterbo. Please be thorough when filling out this form. You will need to have your insurance information handy when filling out this form.

 

All incoming students, whether freshmen or transfers, are required to have a physical. The physical is valid for your entire time at Viterbo University. Only physicals completed after April 1, 2021, will be accepted. Physicals must be completed in person by an MD, PO, PA, or NP. No other providers completing the physical will be accepted. The Viterbo University physical form is the only physical form that will be accepted (found here). There are NO exceptions!! 

 

Athletes that turn forms in after the due date may be subject to a delay in team participation. You must have the form satisfactorily completed, submitted to the Athletic Training Staff, and reviewed before you will be deemed medically eligible to participate in team-related practices, workouts, games, matches, or activities. There are no exceptions.
